Body horror tends to go one of two ways. It is either incredibly visceral, violent, and downright nasty; staying with you long after you have finished watching it and implanting a world of new fears in your mind regarding the many horrifying ways the human body can be tortured. Or it is simply a bit boring and misses the mark by quite a way.

2024 saw a bit of a revival of the body horror genre and that seems to be continuing into this year. With that in mind, we are going to take a look at 10 Brutal Body Horror Movies from the 2020s. You should know what to expect here: violence, gore, brutality, and some seriously twisted examinations of just what can be done to the human anatomy to make viewers wince in terror. 10. Control Freak (2025) – An Itch That You Simply Cannot Scratch

Why it Ranked: We will kick this list off with a movie that I had high hopes for but was sorely disappointed by. Control Freak is, unfortunately, a bit of a misleading movie. While it does occasionally lean into the body horror, it never quite delivers on the genuinely visceral, gnarly gore and nastiness that fans of the genre have come to expect. It frequently ventures off into supernatural horror territory and is weighed down heavily by its overly on-the-nose message. Still, Kelly Marie Tran is on top form, so that is a big bonus.

Synopsis: Valerie is a successful motivational speaker who finds her life unravelling when she develops a persistent, maddening itch on the back of her head. As she scratches until she bleeds, she begins to suspect that the source of her irritation is a parasitic demon rooted in her own family history.

9. Nightbitch (2024) – Motherhood Becomes Feral In Suburbia

Why it Ranked: This is another movie that I was very excited for but ended up being rather disappointing. Amy Adams is absolutely fantastic here, which is something of a crying shame. Around the midway point, it becomes very clear that director Marielle Heller has absolutely no idea where to go with the story. The script quickly becomes laughable and the once intriguing body horror elements fade away to reveal a dull metaphorical drama about parenthood. The humour completely dries up, which is a massive shame because Nightbitch had bags of potential.

Synopsis: A stay-at-home mother, exhausted by the repetitive routine of looking after her toddler, begins to notice strange physical changes. As she grows increasingly feral and develops canine instincts, she must decide whether to fight the transformation or embrace her new, wild self.

8. Together (2025) – Bonding To An Absolute And Horrifying Extreme

Why it Ranked: We have a bit more of a Hollywood take for you here as real-life couple Dave Franco and Alison Brie headline Michael Shanks‘ feature-length movie debut. There are plenty of gruesome moments to make your toes curl and the central idea, focused on matters of co-dependency, has been creatively turned into a fairly visceral and nasty horror concept. It makes for some extremely interesting visuals. There is ongoing litigation regarding this film alleging that the plot was stolen, so it will be interesting to see where that goes.

Synopsis: A struggling couple moves to the countryside to salvage their relationship. During a hike, they stumble into a mysterious cave system where an encounter with an unnatural force causes their bodies to gradually and painfully merge into one single entity.

7. Old (2021) – A Lifetime Of Decay Compressed Into A Single Afternoon

  • Director: M. Night Shyamalan
  • Cast: Gael García Bernal, Vicky Krieps
  • Runtime: 108 minutes
  • IMDb: 5.8/10

Why it Ranked: M. Night Shyamalan occupies that strange middle ground of making rather droll and dull horror movies that critics don’t like but cinema-goers seem to enjoy. While the script is weak and some of the performances are off, I am willing to give the issues a pass because the scenes of characters rapidly ageing are actually incredibly unsettling. If you wanted to condense all of your upcoming lifetime’s existential dread regarding getting older into one near two-hour movie, Old may just do the trick.

Synopsis: A family on a tropical holiday discovers a secluded beach where they begin to age rapidly; reducing their entire life spans into a single day. Trapped by the environment, they must find a way to escape before they die of old age before sunset.

6. Grafted (2024) – A Bloody Pursuit of High School Popularity

Why it Ranked: Grafted is a 2024 Shudder original from New Zealand that probably isn’t going to blow too many people away but is still pretty enjoyable. Around a third of the way in, it really goes off the rails in an extremely enjoyable, very whacky manner. There are some very nasty scenes for body horror fans and the concept at the centre of the story makes for some fantastic, and unexpected, moments. It is a lot of fun but not overwhelmingly brilliant.

Synopsis: An awkward Chinese student studying medical research in New Zealand becomes obsessed with curing her own visible birthmark. Her experiments soon take a dark turn when she discovers a way to “graft” the skin of others onto herself to achieve the perfect look.

5. Huesera: The Bone Woman (2022) – The Fracturing Pressure Of Cultural Expectations

Why it Ranked: Huesera: The Bone Woman is yet another metaphorical drama hiding behind a body horror mask. This allegorical story is placed up against a backdrop of some very nasty and brutal scenes of bones cracking, breaking, and splitting. It might be a bit too slow for some and others may simply dislike the parabolic nature of the story, but it remains an incredibly well-reviewed slow burn horror that is worth watching for its visceral sound design and haunting imagery.

Why it Ranked: I probably only need to mention the fact that this film is directed by David Cronenberg for you to know that it is a weird and trippy body horror. Much like Brandon Cronenberg‘s Infinity Pool, Crimes of the Future takes the audience to a dystopian reality and explores new, depraved ways to exploit the human body. It’s fascinating stuff and some of the body horror elements are particularly nasty. If you are just there for the body horror, this should definitely scratch that itch.

Synopsis: In a future where the human species is adapting to a synthetic environment, the human body undergoes new transformations and mutations. A celebrity performance artist publicly showcases the metamorphosis of his organs in avant-garde performances.

3. Titane (2021) – A High-Octane Nightmare Of Metal And Flesh

Why it Ranked: Titane is unbelievably weird… Like, seriously unbelievably weird. This movie is nuts! I cannot go into it too much and even writing the synopsis was difficult due to the potential for spoilers. All I can say is that Titane is an incredibly uncomfortable movie that takes body horror in a whole new twisted direction. Expect violence, gore, intense scenes, a ridiculous amount of nudity, and cars; but not cars in the way you might expect. Enjoy!

2. The Ugly Stepsister (2025) – A Vile Body Horror Spin On A Classic Tale

Why it Ranked: The Ugly Stepsister is a brand new entry to the body horror genre and another great example of how the 2020s are reviving the sub-genre in a massive way. Its satirical take on the old-fashioned fairy tale is absolutely hilarious while also being rather shocking in parts. This is a body horror that pulls no punches and delivers in all the ways you might expect, so prepare to be frequently surprised by copious amounts of nasty surgery and nudity.

Synopsis: In this comical and grotesque retelling of Cinderella, a young woman goes to immense, surgical lengths to capture the attention of a prince who only has eyes for her stunningly beautiful stepsister, leading to a bloody examination of vanity.

🧬 Flesh and Bone Honourable Mentions

The 2020s have been an absolute goldmine for fans of biological decay and surgical nightmares. While the top 10 covers the heavy hitters, these four entries came dangerously close to making the cut and are essential viewing for anyone who likes their horror visceral.

Possessor (2020): Brandon Cronenberg proves that the obsession with bodily invasion is definitely genetic. This sci-fi horror follows an assassin who uses brain-implant technology to inhabit other people’s bodies. It is cold, clinical, and features some of the most creative and gruesome practical effects of the decade. The “face mask” scene alone is enough to earn it a spot on any body horror fan’s watchlist.

Hatching (2022): This Finnish gem is a masterclass in using body horror as a metaphor for familial pressure. A young gymnast finds a mysterious egg and decides to nurture it, only for a grotesque doppelgänger to emerge. The physical transformation of the creature is truly unsettling, blending bird-like traits with human anatomy in a way that feels both tragic and terrifying.

Infinity Pool (2023): Another Cronenberg entry, this time taking us to a hedonistic resort where guests can create “doubles” of themselves to take the fall for their crimes. The film is a trippy, psychedelic descent into depravity that uses the concept of biological cloning to explore the complete loss of self. It is messy, mean-spirited, and brilliantly weird.

Birth/Rebirth (2023): A modern, feminine take on the Frankenstein mythos. This film follows a morgue technician and a grieving mother who team up to keep a dead child alive through increasingly gruesome biological experiments. It’s a grounded, intense look at the lengths people will go to cheat death, focusing on the raw, unglamorous reality of maintaining a biological miracle.

Why it Ranked: Honestly, there was no other movie that could possibly take the number one spot other than Coralie Fargeat‘s 2024 masterpiece. When it comes to everything you want from a body horror, The Substance delivers in spades. Absolutely horrifying mutations of the human body, extremely nasty scenes depicting graphic wounds, and a whole lot of gore. It is an absolutely hilarious and incredibly apt commentary on the ageism of Hollywood. Demi Moore is sensational. This is a must-watch.
